Although we provide services regardless of a patient’s ability to pay, we are not a free clinic. We offer a sliding fee scale to our uninsured patients based on their household income and the number of people in their family. We accept Medicaid, Medicare, Insure Oklahoma and most private insurance.
If you are unable to keep your appointment, please call our front desk at 918.622.0641 at least 24 hours in advance. This allows us to reschedule your appointment to better meet your needs.
As of July 1, 2024, patients will only be allowed four (4) appointment “no-shows**” in a rolling year or within the last 12 months. After the fourth no-show, patients will become inactive and will be considered a “walk-in basis” patient only. Once patient is seen, patient will be returned to Community Health Connection’s (CHC’s) standard scheduling practice.
Established patients should be able to see a provider the same day if they call or walk-in early enough in the day (pending availability). New patients need to call 918.622.0641 and schedule an appointment.

Insured patients will be responsible for co-pays, deductibles and any lab fees. Uninsured patients pay on a sliding fee scale based on their household income and household size. The sliding scale determines the cost of your office visit.
1. A photo ID.
2. If you have private insurance, Medicaid, Medicare or Insure Oklahoma you must bring your insurance card.
3 If you are uninsured, you need to bring two most recent pay stubs, unemployment check stub or a recent tax return. If you are unemployed, please have a notarized letter stating unemployment.

5. Immunization records (for children needing immunizations).
All Minors (under the age of 18) must be accompanied by an adult if they are coming in for a primary care visit. Please note: Any child under the age of 18 can receive reproductive health treatment and/or behavioral health services without parental consent.
No. CHC is a primary care clinic, not an emergency care facility. Our primary focus is preventive care.
If a patient has been seen by a health care provider at CHC and requires a medical service (i.e. medical procedure, visit to a specialist, etc.) that we don’t provide, CHC’s referral department will assist the patient in scheduling an appointment with a specialist.
Yes. Patients who have the proper documentation and who qualify for the sliding fee scale (patients without insurance), there is no fee. If a patient without insurance does not bring proof of income, or does not qualify for the sliding scale, they will be charged a $10 administration fee. If you have insurance, your insurance should cover it (check with your insurance company for any questions regarding coverage). If you bring in your child for an office visit, in addition to immunizations, the office visit would be a separate charge.

We have Patient Assistance Program for patients who qualify based on income to receive certain medications at no charge. Other medications can be purchased at a discounted rate through our 340 B Program.
Yes. Over 85% of our staff is bilingual in Spanish, including security personnel, front desk staff, nursing staff, dental and medical providers.
